Do You Qualify?
Five questions, about a minute. It will not tell you that you qualify, because only a certified Florida physician can decide that. What it can tell you is whether the appointment is worth booking.
The five questions
Residency, age, your diagnosis and whether you can attend in person. Two of the five can rule you out on their own, because they are hard requirements of the Florida programme instead of matters of judgement.
Why it will not just tell you
Plenty of sites will happily announce that you qualify. They are not in a position to know, and neither are we. Under Florida law a qualified physician makes that determination after examining you and reviewing your history, and a web page announcing the outcome in advance is doing something it should not.
The honest version is narrower and more useful: some answers are hard requirements, and the rest is a conversation worth having. The quiz tells you which of those you are in.

The part most people miss
Florida lets a physician certify a condition of the same kind or class as those on the published list. An unlisted diagnosis is therefore not an automatic no, which is why the quiz answers "it depends" more often than a strict checklist would.
The other one is geographic. Because the first certification has to happen in person, where you live decides how hard this is, and Fort Myers is one of the easier places in Florida to arrange it.
Common questions
No. Florida requires the first certification to be a physical examination conducted in person by a qualified physician. Telehealth is allowed for later renewals with the same physician, but not for the initial appointment. This is the single most common thing people get wrong, usually because national services advertise an entirely online process that is not available to a first-time Florida patient.
Enough that the appointment is rarely the hard part here. The state's Qualified Physician List covers the whole of Lee County, and the clear majority of the physicians on it practise in Fort Myers itself, well ahead of any other city in the county. The exact figures are printed on our directory page beside the list they describe, because the state republishes that file every Friday and a number typed into a sentence here would be stale within the week. The list is alphabetical: we do not rank physicians and nobody pays to appear on it.
